Only 40 Cards are available for adults only on a 
First Come – First Served basis
NO APPOINTMENTS 

Either of the following valid identification are required to obtain a card:

OPTION 1:
Any government issued photo ID, such as a driver’s license AND
either a Saskatchewan Health Card or an original Birth Certificate.

OPTION 2:
A completed and signed guarantor form AND
a photograph that is also signed by the same guarantor AND
either a Saskatchewan Health Card or an original Birth Certificate.

Quint’s Core Neighbourhood at Work staff are available to assist you anytime before the Clinic day with preparing the Guarantor document.
